There has been increased interest in Potential Failure Mode Analyses following the methods’ failure to inform owners and regulators in ways that would have resulted in a successful intervention prior to the February 2017 Oroville dam safety incident. The purpose of this full-day workshop was to review this, and other case studies, review and discuss the strengths and weaknesses of the PFMA method in view of the findings of the Oroville incident Independent Forensic Team (IFT), and to present ongoing work by FERC and others in advancing failure modes analyses.
